Students and Staff:
We are introducing a new health screening process, effective June 1. Please use the form below to complete the online screening tool.
You will need to open this webpage and respond to the question each time you visit our campus. Upon arrival, you’ll be asked to show the “completed form” on your personal device in order to enter the building. Note: This is not an app. It must be accessed through a web browser.
To complete the health screening:
- Be prepared to enter your Student ID# or Employee ID#
- Answer the health screening question
- Click Submit
- Show your screen upon entry
Results will display immediately on your device. Please answer each question honestly.
A “green screen” will indicate that you have passed the health screening for that day and are checked in to be on-site. Please be prepared to show your “green screen” when arriving on campus.
A “red screen” will indicate that you have not passed the health screening for that day and that you should stay home. There is no penalty for failing a screening. If you are experiencing symptoms, do not come to campus and be sure to follow-up with your instructor. Failing a screening prevents you from entering DPS buildings and schools. For health guidance, please reach out to your healthcare provider.
If you do not carry a personal device, you may seek assistance when on campus to complete the Safe Entry screening tool.
